The CLEAR study
Given the decline in screening uptake, our study explores how offering choice in screening positions, including the left lateral position (lying on your side), affects comfort, inclusivity, and engagement.
We aim to understand how and why changes work, and who benefits most, so services can be safer, more patient-centred, and accessible to all.

CLEAR study animation
This short animation explains the different positions you can choose for a cervical screening test.
Everyone is different.
But everyone with a cervix, over the age of 25 no matter what they look like or how they identify… is advised to go for cervical screening and not put it off.
Cervical screening is important because it can find a common virus that can cause changes in the cells of the cervix.
If these changes are not treated, they could turn into cancer later on.
Some people feel embarrassed or worried about the test.
This might be because of negative experiences in the past or because they think it will be uncomfortable.
We’ve learned that when people can make choices about how the test is done they feel more in control and more at ease.
Did you know you can now choose the position for your cervical screening test?
You can choose to lie on your back with your legs bent up and parted or you can lie on your left side with your legs slightly apart, resting on the bed.
Lying on your back might be physically difficult or cause anxiety.
Some people find it more relaxing to lie on their side, which can help make the test feel more comfortable.
Ask your doctor or nurse about your choice of positions before or during your appointment.
They will be able to guide you through the process.
Your health is important so we want your cervical screening to be as comfortable as possible.
You can choose what works best for you.

The Facts
There are 3,000 new cases of cervical cancer per year in the UK
Cervical cancer is curable when diagnosed and treated fast
Screening helps diagnose cervical cancer, but uptake is at a 10 year low
One Solution
The issues stated above contribute to the reducing numbers of people attending screening. The 2020 national guidelines for cervical screening included the left position, and all people should now be offered both positions in their appointment.
Potential benefits of lying on your side:
– More effective for accessing the cervix
– Less exposing
– May be more comfortable
“Even despite having lots of experience of now being in gynaecological care, it actually felt really nice that I was given the option. I could actually just give myself a hug, I suppose, while, you know, while it was happening”
